Here's a pic I took of a giant kingfisher. It was taken at distance with a high zoom. It's about the size of a crow at 45cm long.

I'm would think the most likely reason for the kingfishers demise is being chased by another female,females will kill others in their territory's and if you see one chasing another(and another,I've seen three birds a few feet apart)they will be females,the one in front will be the interloper,cacking itself.
