Environment Agency catch return booklet for anglers

Does anyone know how to locate a copy of the EAs catch return book for anglers? I believe it's can be used by both coarse and game anglers.

I've done a search of the EA site and haven't found anything about it.

If the booklet is from the EA email them for places of where it will be.

Thanks for the advice. I did some further Googling and I found a copy of a couple of sheets from the EA Anglers Logbook.

It's a comprehensive system but it would take a lot of time to analyse a big batch of records.
